Establishing a new team through online registration:
Follow the links through the online registration process to register a new team. Once the team is registered,
the team captain will be sent a link via e-mail to allow him or her to manage the addition of new team members
online. The captain will also be able to send team members a link to the online registration page which will
allow them to pay whatever portion of the team fee the captain determines.
All players must sign the SCORE INDOOR SPORTS PLAYER REGISTRATION AND WAIVER (download here).
If the player has previously registered online, it is not necessary to fill out all of the contact information. Simply
indicate the division, team, player name, then sign and date the form. The team captain must collect and turn
in all player waivers before the first game.
THE TEAM FEE MUST BE PAID IN FULL PRIOR TO THE SECOND GAME.

Fill out the Team Registration Form (download here) and either mail or fax it to SCORE INDOOR SPORTS.
Once we receive the form, we will enter the team into the online system. The captain will still be sent a link via
